What We Learned
Seven published case files entered the Field Investigation this week.
One primary signal was assigned to each file. No double counting. No causal claims.
This is an editorial field-signal distribution based only on the seven case files published during the investigation window. It is not organizational research, not a community-response sample, and not a population estimate.
The coding method was simple:
Category count divided by seven. Rounded to the nearest whole percent.
- Human Glue Dependency™: 29% : 2 of 7 files

- Accountability Breakdown: 14% : 1 of 7 files
- Decision Latency™: 14% : 1 of 7 files

Human Glue Dependency™ leads the distribution for the second consecutive window.
The symptoms changed.
The structure did not.
Flexibility became a workaround. Meetings became performance. Consensus became delay. Work became orphaned. Culture language replaced installed behavior. Emotional volatility shaped the operating climate. Strong results concealed weak systems.
The conclusion held:
Organizations are compensating for missing Stability Infrastructure™.
The notable shift is equally important.
Decision Latency™, which led the August 24 editorial distribution at 43%, receded to 14% this week. Leadership Drift™ and Leadership Alignment Gaps™ entered the distribution for the first time.
The investigation is moving.
From execution mechanics toward leadership behavior.
From visible delay toward the deeper gap between declared values and installed architecture.

The Charge: Guilty By Association™
Ecosystem step: The Charge.
Every file begins with the same question:
What has the organization normalized, tolerated, or protected long enough for instability to become operational?
That is Guilty By Association™.
Not blame.
Evidence.
If leaders tolerate unclear ownership, the system becomes associated with unclear ownership.
If leaders reward heroic intervention, the system becomes associated with dependency.
If leaders praise alignment while decisions remain stalled, the system becomes associated with performance instead of truth flow.
The core verdict question remains:
Is the organization guilty of tolerating instability?
This is where the smoke detector matters.
A smoke detector does not cause the fire. It makes the invisible visible.
The first signal is not the failure. It is the warning that the operating environment requires inspection.

Exhibit B named Implementation Failure™ as a co-dominant signal. It was coded by the title theme of accountability theater to preserve one primary signal per file.
Exhibit D followed the explicit friction chain:

Exhibit E was coded by its values-versus-architecture theme.
Exhibit G was coded by its dependency mechanism: results were produced through intervention and heroics rather than reliable systems.

- Reveal™: Phase 2. Blue and dark. Forensic. Ask, “What am I actually looking at?” Use the Field Guide™ to identify signals, recognize patterns, analyze Stability Drift™, follow the Drift Progression™ Ladder, and complete the ROI Leakage™ Audit.
- Repair™: Phase 3. Purple. Aggressive. Ask, “What evidence do we have?” Use the Investigation Journal™ to record evidence, observations, actions, case files, and historical patterns.
